Help With Response Cards

I am trying to figure out the wording for my response cards. We are giving several choices for dinner (beef, chicken, and fish). One the response card, I would like to know what each person is choosing, so I can put a little card on the inside of their place card with that choice (to indicate to the server what the person is having). Has anyone had luck with a creative way to do this? Should I have each person`s name printed on the card and they can fill in their choice? That creates a lot of work, so if anyone has an alternate idea, I would love to hear about it!
