I Need Help On A Mother Daughter Poem!

Hi, I am having a "Sweet Sixteen" and unfortunatly my mother passed away when I was pretty young! I wanted to have a little poem typed on the back of the program to my party dedicated to her from me, the bad part is is that i am not all that great on writing poems! Please help if you can!
