Addressing Invitations Etiquette

I need to know what is proper etiquette for addressing and invitation when the couple is married and the wife is a physician and the husband is not .
How should I address the envelope???
Dr. Joann Smith and Mr. Joe Smith
Mr. And Dr. Joe Smith
HELP !!!!

Dr. Jane Smith and Mr. John Smith, printed on separate lines.

For inner envelopes, Dr. Smith and Mr. Smith would be printed on the same line.

Mr. John and Dr. Jane Smith is proper.
