Change My Name Back To My Birth Name After Divorce

I`ve been divorced now for 13yrs. I kept my married name for my childrens sake, but now i want to change my last name back to my birth name. How can i do that and when i was divorced i lived in the state of Indiana and now i reside in the state of Georgia, how can i do that? please help me
