This is the one I`m using for mine. I got it of the internet but I don`t remember where. It`s in spanish but if you want the english version you can email me at brenda16@quik.com. This is how I found it:

Yo, Linda
1525 Orangethorpe
Anaheim, California

It looks better centered. (This is not the real thing that I used its just how I got it off the internet.)
Hope I helped.

The phrase we are using for our daughter`s english version is: Princess (your name), summons you to an afternoon of make believe land.... Everyone will be royally feed and happy for the stay.... At Princess (your name) SWEET 15 BIRTHDAY CELEBRATION!!! King (father`s name) and Queen (mother`s name) request the honor of your presence for the church ceremony at (fill name and location of church), On December 10, 2005 at two o`clock in the afternoon. The Royal Ball is at 5 p.m. At the Castle of the (name and location of hall).
This all went down right side of the invitation and a baby photo of my daughter with flowers around it (or your theme) was placed on the left side of the invitation. Then we made programs, like a little phamphlet (trifold) of what`s going on for the church ceremony on inside left flap listed church events like; entrance, first reading, second reading, dedication ceremony lighting of the candle, and listed what will all take place and the rest of church service with songs. Then the reception was listed in the middle with all the damas, chambelanes, padrinos de baptismal, padrinos de confirmation, padrinos and their gifts, grandparents, parents, quinceanera and her escort. On the right flap we listed the celebration of events like the dinner, prensenting of last doll, crowning, changing of shoes, fathers waltz, quince waltz, court dance, all dance, serenade, cutting of cake, toast, pinata, acknowledgements by quinceanera, final song, and thank you. That all went inside on the outside was front cover was name of quinceanera w/photo and date of cumpleanos, quinceanera prayer on left flap, and a statement from our daughter titled to my parents on the back of program. Well that`s what we did hope it gives you kind of an idea of what to do. Good luck and feliz cumpleanos!
Spice it up, change or add things the phamphlet is also a recuerdo of the day for your guests.
