5 Course Dinner ; Game

I`m looking for a dinner menu where the guests pick what they want for several courses. They have a sheet of paper with discriptions..example Popeye`s choice would be an olive. They choose 3 items per course and depending on what they pick they could get something like a toothpick, a cup of soup and a knife. Does anyone have this game?
