I like your idea with the white tablecloth and red and blue napkins. Here would be my suggestion for centerpieces:

Search on the web for blue and red tin pails. They are on lots of sites. Then get some red, white and blue shredded tissue paper and fill the pails. In the pails, place a small (miniature) American flag - the kind they give out at parades along with a small (mini) MYLAR balloon (the kind that are in floral arrangements) that says "Happy 40th Birthday" (if there is one) or just "Happy Birthday". You can find the little flags on lots of sites. A good one is orientaltradingcompany.
Go to any local florist for the mylar balloon. If you think you need more, you can also get some red and blue glass votive candles. These can also be found on lots of sites including ebay.
