Is QCMaidToOrder Bridal A Fraud

A friend of mine is getting married and she received an email from a guy saying they had her dress and emailed her a form to fill out. On the form had this website address: http://www.bridesweb.com/MaidToOrder.htm

Is this a valid vendor of bridal gowns? Can this site be trusted? Can anyone vouch for this site?
