(15th) Birthday Party Restaurant

Does any body know the name of the restaurant that gives out the tall white chef`s hat and then the people write something funny on the top of the hat??? (in illinois)

Or does anyone know any good restaurants in chicago or near chicago to have a good time for a 15th birthday party??
