Tipping

Who expects a tip, and what is an appropriate amount? The DJ, photographer, limo driver..(.and does it make a difference if the person owns the service or is an employee of someone else)? Also, not to sound totally ignorant, but are we expected to pay for dinners at the reception for the DJ and the photographer?
