I Need Small Champagne Bottle Labels

Perhaps I could design some labels to fit the bottle. What are the dimensions of the label? What do you want the label to say? How many do you need? When do you need them?

They should be 2"1/2 or 3" wide by 4 inches high (estimated). I have 150 bottles and I want them to say :
