Graduation Announcement Wording Help

My niece is graduating from high school, and we have some questions about wording. She has a different last name than her mother and her grandmother, both who will be providing her with addresses of people to whom she should send announcements.

How would one identify her to people who would get the announcement and not recognize the name?

Would it be appropriate to enclose a note that says "Jennifer is the daughter of Sue Jones and the granddaughter of Ann Smith?"
