Names On Placecards

Help. One of our wedding guests recently passes away. How do I write the placecard for the widow? Do I put Mrs. (man`s name) LastName or Mrs. (her name) LastName? Which is more appropriate. I don`t want to offend anyone.

When writing out a widow`s name, it would read
Mrs. (John) Doe
Using the woman`s first name would indicate that there was a divorce.
