How about baby bingo? You make up a bingo sheet and make the center the "free space" with a baby themed picture. Then you have the guests scribble down what they think the mom-to-be will receive as gifts. The first few to get bingo win a little prize. This makes it fun for the guests watching lots and lots of presents being opened, and they won`t lose interest. I have one I could email you.

Another idea for the cake is edible images. Do a collage of the mom to be in her different stages of pregnancy on the cake. Or if the mom-to-be is camera shy, just create a nice baby themed image. Http://www.sweettreatphotos.com.
