At this time, we are specifically seeking a bride and groom who are planning to be wed within the next 4 months and are both interested in having an Extreme Makeover.
Extreme Makeover offers selected candidates a truly Cinderella-like experience by altering their appearance through plastic surgery in an effort to transform their lives and destinies.
This is all accomplished through the skills of our "Extreme Team," which includes the country’s finest plastic surgeons, eye surgeons, and cosmetic dentists along with a talented team of hair and make-up artists, fashion stylists and personal trainers.
Remember, this is "Extreme Makeover." Applicants should want more than one procedure be it rhinoplasty, porcelain veneers, a facelift, chin or cheek implants, breast augmentation, etc.

My name is danielle leach im 19 years old, my fiancee is 20 we planned on getting married march 26th 2005, but im not ready to be seen in a bridal gown without confidence in myself inside and out. We have came along way and now we need your help. We also have been together since january 6th 2000, we have four children so you can imagine what i need done. If we got picked for your show i would like a tummy tuck, breats lift, and lipo scuction, and some dental work as well. But i just need to know where do i fill out an application and what are the qualifcations.
